Okay, do you need just a listing of diagnoses and codes, or do you need the diagnostic criteria for each?

As far as the submission process ... I haven't seen my full size DSM-IV (aka "The Big Book of Crazy") since I took Psychopathology in grad school. I keep the "desk reference" (a wirebound mini version that has all the criteria and the codes, but leaves out a lot of the extraneous information). I also have the DSM-IV Casebook, which gives you a case presentation and then you have to figure out the diagnosis and differential diagnosis.

I checked the desk reference and it doesn't discuss the submission criteria. I kinda remember reading about it in the full size edition at one point. If I can't find mine I can check it at work tomorrow.
