It's for joining sheets of paper together without using staples. We had something similar at school (I really am getting old). When you hit the knob it'll cut a hole like a normal hole-punch only longer and straight, next to that it cuts a little tab which you then fold up inside the hole to hold a few sheets together.

I remember those devices. Sometimes we did it by hand, in lieu of a stapler and in a pinch.

Which makes me want to be all OCD once again and remind people that THIS is how you staple papers, and why:

My wife bought one of those and more than once she has sent me to work to scan documents that have had key things like dates or other information obliterated by this contraption. Good thing is that if you are really careful you can unfold it and put it back together to scan.

Reminds me of one of those lunch order ticket holders you would find in an old deli. But the base looks like an ash tray. Maybe it's a combo?

It's an old laboratory instrument for researching the effects of cigarette smoke on mice which were suspended in clips on their tails over an ashtray containing multiple lit cigarettes.
