Emotionally, there's a difference between pre- and post-surgery. If it were me, I'd hate to have to tell people about it while surgery was still in the future, because they're going to worry, and wonder if it will go well, and want to talk about it when in fact there's nothing that can be done. Much easier to tell people, "Oh, this? Yeah, I had surgery for a melanoma (or whatever.) It's all going to be fine though," once there is some evidence that it is actually all going to be fine.
