Mac Photo / Signup software?

I'm looking for some software for mac.

Runs in "Kiosk" mode
The user pushes a button, and the built-in camera snaps a photo of them
they fill in fields with email, phone #, etc
the software then generates a contact list with photos, or a searchable database, or exports vcards, whatever

anybody know of anything that functions like that?
