He believes he'd already had his first stroke when the cat grabbed him, and in fact that the cat had picked up on this and therefore grabbed him and dragged him off stage to protect him. How is a cat supposed to know what else to do? Anyway medically this version can't be authenticated or disproved since Roy had multiple strokes, and the doctors could not determine the time of the first stroke. As we should all know that although Seigfried and Roy have had their last performance, Roy has made a marvelous recovery, well beyond what were the best predictions.
