It looks like we now have a system where people can screw up with noone providing any checks on bad information before the commission of extraordinary acts.

We all know about people freed from prison after being wrongfully convicted, but at least they had a trial and for issues as stupid as a similarity in names, the system will correct itself after someone has been in jail a few days.

How do you correct a situation after kidnapping someone and subjecting them to extreme interrogation? What happens if they die in custody?
