Well, no pain really.
From what I understand they are raising their blood pressure by "pain" in the areas of their body that do not feel it. Being spinal injury athletes the injuries are inflicted below the line of paralysis.

That's what I got from a 5 Live interview with a Paralympic anyway.

the ouch was from me. The "No pain No gain pun was intentional, but alas too subtle.
